What is the rule/etiquette for declaring? Do you do it at the end of an over or can you do it whenever you choose?

Here's a thought. Even in their pomp, England never managed to play pace. They were humiliated by Morkel and Steen not long ago. There really isn't anyone else at the moment so while Johnson was off being useless this was not such a visible flaw. But here we've been undone by it and now we need to learn how to play it.
A specialist footwork coach would be good too. I vote Ramprakash, or Gough.

Only when I know very little about the subject. I have a fairly good but very sparse history of leaving race courses with more money than I arrived with. But, as my performance at bind tastings demonstrates, when I build a significant working knowledge of the subject I over-think the problem and fail miserably.

That's fascinating. This is exactly what Alistair Cook and Andy Flower have done. They've turned up at each ground, looked at it, had a goji berry flapjack, and decided it was a "Noval 66" or "Martinez 70" pitch. In fact it's turned out to be Dow 60 each time.
